Please note the links to our various office forms to the left. At the very least, all new patients (and patients who have not been seen by Dr. Hantke within the last three years) will need to complete the Patient Registration, Health Questionnaire and Privacy Policy forms and bring them to the office at the time of their visit. Existing patients will need to complete the health questionaire at least annually and upon any significant change in their health status. Those patients who either desire a copy of their previous records to be forwarded to Dr. Hantke or desire that Dr. Hantke forward a copy of his records elsewhere will need to print, complete and supply us with a copy of the Release of Information form.
